The nearly impossible challenge to manage severe glaucoma in a patient with extremely deep orbits leaves only one option to manage IOP in both eyes. There was no access for trabeculectomy, ab-externo stent or extraocular shunt placement. The orbit was even deeper in the second case and required a lateral canthotomy and a traction suture just to acquire visualization of the superior limbus.
