A 7 year-old boy with Marfan syndrome presented with ectopia lentis and best-corrected visual acuity of 20/70. We demonstrate lensectomy via an intracapsular approach by which a 23-gauge vitrector was used to enter through the capsule to aspirate the lens contents, followed by removal of the capsular bag. We describe surgical technique for safe lensectomy in cases of zonular instability.
