Acute fluid misdirection syndrome (AFMS) is characterized by a very shallow anterior chamber with the absence of suprachoroidal effusion or hemorrhage. We showed a case started coughing during stromal hydration in the secondary intraocular lens implantation, which induced AFMS. The injection of air bubble was helpful in deepening the anterior chamber without performing a posterior decompression.
