Iris prolapse is a problem during phaco in IFIS. Hydrodissection maneuvers predispose to this. To solve this problem I introduce the concept of sleeve hydrodissection. In this technique I introduce the hydrodissection cannula within the phaco sleeve through the incision and then do the hydrodissection. The phaco sleeve plugs the incision, prevents leak and stops the iris from prolapsing.
