A girl aged 4 years presented with bilateral progressive decrease in distance vision. Slit lamp biomicroscopy revealed microspherophakia. She was managed by phacoaspiration and in-the-bag intraocular lens implantation using capsular tension segments. Postoperatively she had a good visual acuity and a central well placed IOL.
